- Design an efficient pathway for referring physicians to assist patients with getting the appropriate pre-treatment evaluation in a timely fashion and to meet treating physicians in a timely manner.
- Educate primary care providers regarding the referral pathway.
Outcomes
- Will streamline the patient navigation through pre-treatment evaluation so that necessary investigations are arranged quickly and unnecessary investigations and evaluations are omitted.
- Better access to care for patients and reduction of costs for unnecessary investigations.
